This Acknowledgment records the Company property issued to the individual who signs below (“You”) and the terms of its return.

Terms of Issuance and Return
- Issued items remain Company property and are for your use in providing services or performing your job. Do not duplicate keys or share codes or credentials with anyone.
- Lost or stolen items, or any suspected unauthorized access, must be reported to the Company immediately.
- All physical items must be returned on or before your last day of service or employment, or earlier on the Company’s request. Digital access will be deactivated by the Company and does not require return.
Replacement Charges; Authorization
You agree that each physical item above that is issued to you and not returned may be charged to you at the Company’s reasonable replacement and rekeying cost, currently fifty dollars ($50.00) per item. If you are a W-2 employee, you hereby voluntarily authorize the Company, in writing, to deduct such charges from your wages, including your final paycheck, to the extent permitted by law; no deduction will reduce your wages below the minimum wage or cut into earned overtime. If you are an independent contractor, you agree the Company may offset such charges against amounts payable to you, with a written itemized statement provided. In either case, charges apply only to items actually issued to you and not returned, and will be waived for items the Company confirms were returned or that you replaced at your own cost.
This Acknowledgment does not create an employment relationship with any contractor and does not alter anyone’s classification. It is governed by Texas law, with venue in Harris County, Texas.

By signing below, I confirm receipt of the items checked above and agree to the terms of this Acknowledgment, including the replacement-charge authorization.
