This letter serves as a written notice that we will be extending your probation period for an additional ninety (90) days due to some company policy violations and unfavorable activities (policies & unfavorable activities listed below). By signing this document you affirm that you understand this letter. 

• Buring Patient With The Radio Frequency Machine
• Four (4) Unexcused Absences
• Inappriatly Executing of Treatment Which Caused Bruising of Patient (Dr. Elsworth patient) 

You're Extended Probation Period:

It is understood and agreed that this ninety (90) days extension of your probationary period if set forth for the Company to further evaluate your performance and suitability for continued employment.

Employment may be terminated without notice at any time during this period. Completion of this period is also not a guarantee of continued employment. During the entire course of your employment, you are an "at-will" employee, so either you or Carolina Pintos Therapy, PLLC may terminate your employment with or without cause, and with or without notice, as long as it is consistent with any and all applicable laws.
