Pursuant to our recent discussions, we are very pleased to extend to you an offer of employment to join Carolina Pintos Therapy, PLLC as a Post-Surgery Therapist. This letter formally outlines the terms and conditions of your employment with Carolina Pintos Therapy, PLLC.

Homero Miranda or Carolina Miranda

Compensation:

As a non-exempt employee, you will be compensated at $27.00 per hour. Employees are paid on a bi-weekly basis. All wages are subject to deductions for taxes and other withholdings as required by law. Tips are distributed from the tip pool every pay period. All employees who complete a full 12 months of employment will also be eligible for the annual bonus program. Benefits are only available for full-time employees.

The Carolina Pintos Therapy, PLLC bonus award reflects each employee’s evaluation of effort which will be elected by the lead therapist and owners. The bonus will reflect the excellent performance, the contributions made, and the goals achieved on behalf of Carolina Pintos Therapy, PLLC during the past year.

Probation Period:

It is understood and agreed that the first ninety (90) days of this W2 transition period shall constitute as a probationary period during which period the Company may, in its absolute discretion, terminate your employment, for any reason without notice or cause. During this probationary period, your performance and suitability for continued employment will be monitored.

Conditions:

Your employment with Carolina Pintos Therapy, PLLC is “at-will,” and is at the mutual consent of both you (as the employee) and Carolina Pintos Therapy, PLLC (as the employer), and both parties retain the right to terminate the employment relationship at will, at any time, with or without prior notice and with or without cause. At-will employment also means that Carolina Pintos Therapy, PLLC can change your position, duties, title, compensation, benefits, and/or any other terms or conditions of employment at any time, with or without notice and with or without cause. Only the owners (Homero Rubim Miranda or Carolina Pintos Miranda) have the authority, in writing signed by all parties, to make any agreement for employment on terms other than at will. This at-will employment relationship will remain in full force and effect notwithstanding any changes that may occur in your position, title, pay, or other terms or conditions of your employment with Carolina Pintos Therapy, PLLC.

You will be required to execute certain documents (i.e., Confidentiality Agreement, Non-Competition Agreement, etc.) as well as other new hire paperwork on your first day. All these documents will be visible in your employee portal, and we request that you sign all policies assigned to you no later than your 3rd day of employment. In accordance with federal law, you will be required to provide documentation verifying your eligibility to work in the United States within three (3) business days of your commencement of employment. Please be prepared to provide ORIGINAL documents on your first day of work.

Further, this offer is contingent upon your successful completion of a background check and drug screen. Again, we are excited to offer you the opportunity to join the Carolina Pintos Therapy team.
