Model Agreement for Scar Camouflage Tattooing Services

This Agreement is entered into between Carolina Pintos Therapy, PLLC ("Provider") and the undersigned client ("Model") as of the date signed below.

Acknowledgment of Service

I, the undersigned, understand and acknowledge that the scar camouflage tattooing services provided by Carolina Pintos Therapy, PLLC are being performed by Elly Melendez, a licensed esthetician with extensive experience in skin treatments. While newly licensed in the specialty of scar camouflage tattooing, this procedure is offered as a free service to models for the purpose of skill development and portfolio creation.

I acknowledge that the purpose of this procedure is to allow the esthetician to refine her skills and build her professional portfolio. I agree to receive this free treatment with the understanding that I will not hold Carolina Pintos Therapy, PLLC or its team liable for any dissatisfaction or undesired outcomes.

Waiver of Rights to Legal Action and Reviews

Waiver of Negative Reviews or Defamation: By signing this Agreement, I agree that I shall not, under any circumstances, publish, post, or otherwise disseminate any negative reviews, comments, or critiques of Carolina Pintos Therapy, PLLC, its employees, contractors, or affiliates, including but not limited to Elly Melendez, in any public or private forum. This includes but is not limited to online reviews, social media platforms, and personal communications. I further waive any and all rights to publicly or privately defame or criticize the Provider for any reason related to the treatment provided under this Agreement.
Waiver of Legal Claims: I fully release and waive my right to file any lawsuit, claim, or legal action against Carolina Pintos Therapy, PLLC, its employees, contractors, or affiliates for any reason related to this procedure. I understand that I am receiving this service free of charge and that the Provider has made no guarantees regarding results.
Responsibility for Legal Costs: In the event that I initiate a lawsuit, claim, or other legal action in violation of this Agreement, I agree to be solely responsible for all legal fees, court costs, and any other expenses incurred by the Provider in defending against such action.

Assumption of Risk

I understand and agree that there are inherent risks associated with scar camouflage tattooing, including but not limited to:

Changes in skin texture or pigmentation
Discomfort during or after the procedure
Risk of infection if aftercare is not followed properly
Variability in results due to individual factors

I voluntarily assume all risks associated with this procedure and release Carolina Pintos Therapy, PLLC, its employees, and contractors, including Elly Melendez, from any liability.

Consent to Photography and Marketing Use

I consent to the taking of photographs and/or videos of the treated area before, during, and after the procedure. These materials may be used for promotional purposes, including social media, websites, and training. All efforts will be made to protect my privacy, with images focusing solely on the treated area unless additional consent is provided.

No Guarantees

I acknowledge that no specific outcomes are guaranteed and that this procedure is provided as a free service. I accept that results may vary based on my unique skin and healing characteristics.

Binding Agreement

This Agreement constitutes the entire understanding between the parties. It supersedes any prior agreements or discussions and may not be modified except in writing signed by both parties. This Agreement shall be governed by the laws of the State of Texas.

By signing below, I acknowledge that I have read and understood this Agreement in full. I agree to receive this free treatment under the terms outlined above, and I accept all risks, waivers, and conditions.
