I, the client, authorize CAROLINA PINTOS THERAPY, PLLC, and its team, employees, and/or associates to provide the package deal treatment consisting of 6 sessions for slimming, cellulite, muscle toning, or other specified treatments. I understand and agree to the following terms and conditions:

Package Deal Information

1. Trial Sessions: I understand that if I’d like to try a service before committing to a package, up to 2 prior sessions can be credited towards the package price.
2. Deposit Waiver: Upon purchasing a package of 6 sessions, I will no longer need to pay the deposit amount for each session.
3. Missed Appointments: I acknowledge that if I miss an appointment, I will be responsible for the full price of the missed session. I understand that missed appointments will not be refunded, even though the package offers a better overall price.

Rescheduling & Cancellations

I, the client, understand that my appointment is reserved exclusively for me. Therefore, all cancellations or rescheduling must be done at least 48 hours prior to my treatment.

1. Advance Notice: Cancellations or rescheduling within 48 hours of the appointment will result in forfeiture of any deposit made.
2. Last-Minute Changes: I understand that last-minute cancellations or rescheduling will not be accepted. If I cancel or reschedule after the 48-hour window, I will forfeit my right to a refund of the deposit.

Please Don’t Be Late

I understand that appointments are scheduled back-to-back. If I am late, my appointment will be cut short to accommodate the next client.

Client Acknowledgment

I, the client, acknowledge that I have read and understood the terms and conditions of purchasing a package deal. I agree to abide by the policies outlined above regarding trial sessions, deposits, missed appointments, rescheduling, cancellations, and punctuality.

I understand that the package deal offers a better overall price, but missed appointments will not be refunded. I also acknowledge the 48-hour cancellation policy and accept that cancellations or rescheduling within this window will result in forfeiture of my deposit.

I certify that I have had the opportunity to ask questions about this contract, and these questions have been answered to my satisfaction.
