Pursuant to our recent discussions, we are very pleased to extend to you an offer of employment to join Carolina Pintos Therapy, PLLC as a Physician Assistant. This letter formally outlines the terms and conditions of your employment or contract with Carolina Pintos Therapy, PLLC.

Compensation:

As a contractor, you will receive a 50% commission on the net profit of all services provided (total service charge minus the cost of products used to provide the service). Additionally, you will earn a 10% commission on the sale of any cosmetic or aesthetic products. Contractors are paid bi-weekly.

In addition to the above compensation, you will also be compensated for your contributions in creating protocols and assisting in the drafting of documents necessary for conducting all appropriate treatments. For these specific tasks, you will be paid at a rate of $30.00 per hour. This payment will be made in accordance with our bi-weekly payroll schedule and is separate from the commission-based earnings outlined above. This hourly rate reflects the specialized knowledge and expertise required to develop these critical operational materials and protocols prior to your start date.

Probation Period:

It is understood and agreed that the first ninety (90) days of employment shall constitute a probationary period during which period the Company may, in its absolute discretion, terminate your employment, for any reason without notice or cause. During this probationary period, your performance and suitability for continued employment will be monitored.

Conditions:

Your contract with Carolina Pintos Therapy, PLLC is “at-will,” and is at the mutual consent of both you (as the employee) and Carolina Pintos Therapy, PLLC (as the employer), and both parties retain the right to terminate the employment relationship at will, at any time, with or without prior notice and with or without cause. At-will employment also means that Carolina Pintos Therapy, PLLC can change your position, duties, title, compensation, benefits, and/or any other terms or conditions of employment at any time, with or without notice and with or without cause. Only the owners (Homero Rubim Miranda or Carolina Pintos Miranda) have the authority, in writing signed by all parties, to make any agreement for employment on terms other than at will. This at-will employment relationship will remain in full force and effect notwithstanding any changes that may occur in your position, title, pay, or other terms or conditions of your employment with Carolina Pintos Therapy, PLLC.

You will be required to execute certain documents (i.e., Confidentiality Agreement, Non-Competition Agreement, etc.) as well as other new hire paperwork on your first day. All these documents will be visible in your employee portal, and we request that you sign all policies assigned to you no later than your 3rd day of employment. In accordance with federal law, you will be required to provide documentation verifying your eligibility to work in the United States within three (3) business days of your commencement of employment. Please be prepared to provide ORIGINAL documents on your first day of work.

Further, this offer is contingent upon your successful completion of a background check and drug screen. Again, we are excited to offer you the opportunity to join the Carolina Pintos Therapy team.
