Re: Offer for Independent Contractor Role as Post-Surgical Therapist
Following our recent discussions, we are delighted to offer you the opportunity to join Carolina Pintos Therapy, PLLC (the “Company”) as an independent contractor Post-Surgical Therapist. This letter outlines the key terms of your engagement.
Start Date:
07/07/2026 @ 9AM (Memorial Location). Your training program begins on your start date.

Services:
Post-surgical therapy services — including manual lymphatic drainage and related post-operative recovery care — at the Company’s locations and, where you accept them, home-visit engagements. You determine the manner and means of performing the services, subject to applicable law, licensure requirements, patient-safety and HIPAA standards, and facility rules.
Compensation:
You will be compensated at $27.00 per documented service hour, plus gratuities. Gratuities left by your clients are yours and are never pooled: card gratuities are paid to you per the Company’s point-of-sale records, cash tips go directly to you, and Zelle tips are sent by the client to the Company and paid to you in cash. Payments are made bi-weekly based on services documented in the Company’s systems. You may also participate in the Company’s voluntary points bonus program (every 10 points = $50). We typically review the engagement, including rates, annually — or earlier where performance warrants — and any change to your compensation takes effect by written agreement between you and the Company. As an independent contractor, you are solely responsible for all federal, state, and self-employment taxes; you will complete IRS Form W-9 before your first payment, and the Company will issue Form 1099 as required by law.
Training:
You will receive the specialized post-surgical clinical training program described in the Training Investment and Reimbursement Agreement (value $4,000), with monthly forgiveness and full forgiveness upon any conversion to employment, as set out in that agreement.
Transition to W-2 Employment:
The Company’s practice is to consider strong contractors for conversion to W-2 employment, typically after about one (1) year of continuous engagement. Any conversion is at the Company’s discretion, subject to mutual agreement and a separate employment offer, and is not guaranteed. Upon conversion, any remaining training-reimbursement balance is forgiven.
Engagement Terms:
You are an independent contractor, not an employee, and are not entitled to employee benefits, workers’ compensation coverage, or unemployment contributions from the Company. Nothing in this letter creates an employment relationship. Either party may end the engagement at any time with written notice; upon termination, the Company will pay for all services performed through the end date, subject to any offset expressly authorized in your signed Training Investment and Reimbursement Agreement (with an itemized statement provided).
Required Documents:
This engagement is conditioned on your executing the following, provided for signature before or at the start of the engagement: the Independent Contractor Services Agreement; the Confidentiality, Intellectual Property & Non-Solicitation Agreement; the Training Investment and Reimbursement Agreement; and the Facility, Scheduling, Service Documentation & Points Standards.
Contingencies:
This offer is contingent upon your successful completion of a background check and drug screen, and verification of the licenses or certifications required for the services.
Governing Law:
This letter is governed by the laws of the State of Texas, with venue in Harris County, Texas. If any term of this letter conflicts with the Independent Contractor Services Agreement, the Services Agreement controls.
