Re: Acknowledgment of Your Resignation

We acknowledge receipt of your resignation from your role with Carolina Pintos Therapy, PLLC (the “Company”), effective 07/07/2026. We regretfully accept it, and we understand you have decided to pursue opportunities that better align with your goals.

We want to sincerely thank you for your contributions during your time with the Company. Your work, professionalism, and care for our patients have been genuinely valued, and we are grateful for the effort you have given.

Your Continuing Obligations

As a reminder, the agreements you signed with the Company that by their terms survive the end of the relationship — including confidentiality and protection of the Company’s proprietary information, intellectual-property assignment, non-solicitation of the Company’s patients, referral sources, and staff, and, if you signed one, your Non-Competition Agreement — remain in effect according to their terms. Please refer to your signed copies for the specific terms; copies are available from the Company on request. The Company reserves all rights under those agreements.

Return of Company Property

On or before your last day, please return all Company property in your possession — including keys and access credentials, devices, records, patient information in any form, and materials — and do not retain any copies. If you signed a Key & Property Issuance Acknowledgment, the replacement charges authorized in it apply to any issued items not returned, and your digital access will be deactivated as of your last day.

Final Payment

If you are an employee, your final wages will be paid in accordance with the Texas Payday Law — by the next regularly scheduled payday — and will include all earned wages, overtime, tips, and earned commissions and bonuses per the applicable plan terms. If you are an independent contractor, the Company will pay for all services performed and documented through your end date in accordance with your engagement terms; if any offset applies under a Training Investment and Reimbursement Agreement you signed, it will be calculated and applied strictly as set out in that agreement, with a written itemized statement provided to you. Final payment will be made by direct deposit unless you notify the Company in writing that you prefer a mailed check.
