This Signing Bonus Agreement (“Agreement”) is entered into between Carolina Pintos Therapy, PLLC (“Company”) and the undersigned (“Employee” or “Contractor”), effective as of the date signed below.

As part of our commitment to supporting new team members, Carolina Pintos Therapy, PLLC provides a $2,000 signing bonus to all new hires. This signing bonus is earned and payable only upon successful completion of twelve (12) consecutive months of continuous employment or contracted service with the Company.

If employment or the contractual relationship ends for any reason before completing the twelve (12) month period, the signing bonus will not be paid.

This bonus is a one-time incentive and does not alter any other terms or conditions of your employment, contract, or any related agreements with Carolina Pintos Therapy, PLLC.

EMPLOYEE ACCEPTANCE
